I was facing the exact same issue! I had checked (and corrected) that all directories in the path had execute permissions, but the problem remained. In my case, I had to add +read permission to others group for all folders inside public_html. So the mask is 0755 for all directories. In case someone ...

Even though you solved to problem, have a look at php Malicious Code Scanner, which you could schedule as a cron job. Or even Linux Malware Detect.

I agree with ScIT. Moreover, black hat hackers usually launch their attacks through other systems in other countries. So it doesn't really matter where the hacker is located physically. All that matters is the security systems of the server and the awareness of the person responsible for it.
