Thanks for reply! I know how to create a new package, but still this error message will appear "Error: Limit reached / Upgrade package" .
The question now is this, vesta received whmcs to create host command successfully created the account, but whmcs here is the pending state, if the whmcs interface to modify the default package (palegreen, gainsboro, slategrey) name can be used normally, what is the reason? could it be that I have to use the default package name and can not use the name of the newly created package?

Hello,
One of my website went crazy and used 240GB of my 250GB available space on my VPS.
The user had a package limit of 5GB.
Why didn't the limit stop this from happening?
thanks in advance.

Don't you have backups enabled? If no maybe something with quota.
