Options +Indexes

I am getting 403 error with having
Options +Indexes in .htaccess
I have gone all the way up to 644 permissions on .htaccess and the file is owned by the correct user/group.
domain.com/subdir/ = 403
domain.com/subdir/textfile.txt = display
I went away for a bit to clear my head and when I came back I am correct on the above.
I have a separate user where .htaccess works perfectly with both 400 and 644 and I get the directory listing.
Not sure? Everything seems to match as should be.
