Exim + ClamAv controlling local mail?

Exim + ClamAv controlling local mail?
Hello everyone, forgive this account with 0 post but I have not managed to recover my previous account in the forum.
I need a little help with this topic, because it is the first time and I don't know if I can do this configuration.
I have a VPS server with VESTACP with the following configuration:
Apache + Nginx + Exim + Dovecot + SpamAssasin + ClamAv
So far so good, but I have the following problem:
When I send an email from an external account to the server, be it gmail or an external corporate domain to an email account from the server: if I send the EICAR test file for CLAMAV, the email never comes by CLAMAV control, so far everything is perfect and this is very good to control old or already detected viruses.
The problem is, if I send an email between the server's email accounts, the alert does not jump and it is sent correctly between local accounts, this is a security flaw that I don't want to have in the system, but I don't know how to indicate it exim or clamav that also controls local emails.
Can anybody help me?
