Hello.
I am receiving this error last few months. Before that my Server ran smoothly last ~5 years without any problem.
I am able to receive emails fine, but I cannot send to outlook or gmail anymore.
When I hit send in Roundcube it gets stuck in Message Queue for some time, then Mail Delivery System sends me email saying:
"Mail delivery failed: returning message to sender"
I tried reinstalling Server OS. I even tried HestiaCP, but same result.
What could I be missing?

I think the problem is not in the Exim service as with both VestaCP and HestiaCP you had the same error.
And even re-installed the server.
Searching the internet for this error "599 eof" can't find anything about it.
As the problem only happens with Hotmail and Gmail, I think there is some specific configuration in your installation that is causing this, maybe something related to the infrastructure or firewall.

Got a new IP in the same block from my VPS Host, 100% clean on mxtoolbox. but didn't work, same error.
Then got a new VPS (new hardware) from my VPS Host, with another new IP but in different block, 100% clean on mxtoolbox. but didn't work either, same error.
what is wrong :(

The error "Timeout waiting for client input" indicates that it is something related to the network, firewall or even a block of the IPs you are using.
Unfortunately, being OK in mxtoolbox is not a guarantee of delivery for Gmail and Hotmail, as they use their own criteria.
if emails are normally sent to other domains, then the problem is with Gmail and Hotmail and it would not be an error on your Exim.
Try contacting Gmail and Hotmail to see if they help. Talk to your VPS host to have them check this out.
