Unable to add domain to Vesta

After installing vesta i am stuck at adding domain to my server .( i have worked with dns/web/domains in the past, this looks like a simple task but its not working now . i don't know why ? maybe my brain is fried .
i have tried may time followed all tuts .no result . help me .
1. how do i add a domain to my account ? (the most confusing thing is with nameservers /dns records) ?
2. how do i add a subdomain.mydomain.tld this mydomain.tld in using cloudflare i tried setting up a record for subdomain to my server ip . but it didn't work .
