Password "Missing"/Fail to connect to DB

Hello.
I am having some problems regarding the user database password.
Sometime after setup an username and password i lost connection to database with those credentials.

To be able to connect again i need to save that password again to connect, but then i lost connection after a while.
What can be causing that problem ?

my.cnf conf ?
/var/log -> mysql log ?
