can't add mysql database

Greetings,
I can't add database , field host is blank.Also rouncube give err

DATABASE ERROR: CONNECTION FAILED!

Unable to connect to the database!
Please contact your server-administrator.

solutions to fix.. i reinstall 2 times .. no fixed.

reinstall on clean server was the solutions.
