I'm using Ray-Mon, it's up to you how you write the front-end HTML part, I'm using it in a way that users can see the server status publicly. Also there's another one called Yahei-PHP Prober, it's showing live details about your server and PHP installation, you can execute tests and mach your server with other hosts.
